Once you choose a specialist and have scheduled an appointment, you can request a referral by:
To process your referral accurately and timely the following information is required:
- Patient’s name
- Patient’s date of birth
- Patient’s telephone number
- Specialist’s name
- Specialist’s telephone number
- Specialist’s fax number
- Specialist’s NPI
- Date of appointment
- Reason for appointment (diagnosis)
After seeing a Specialist, please forward any records/care summaries if seen by an outside organization.
If you do not have all of this information, please contact the specialist's office to obtain the needed information prior to submitting your request.
